A few years ago, the only art cinema in my town screened "The General". Music was provided by a professional organist, using the theater's own excellent instrument (able to produce some sound effects!).

I asked him about the music, and he told me that he had watched a DVD of the movie several times, and then selected music that he thought would be appropriate--the first time the locomotive came on-screen, for example, he played a snatch of "The Atcheson, Topeka and the Santa Fe."
